This research is descriptive non-experimental with retrospective data collection. The data is taken from the medical records of outpatients who met the inclusion criteria. The detail of oral antidiabetic cost has obtained from the pharmacy installation. Conversely, the cost of laboratory tests and physician services obtained from the finance department. Patients who met the inclusion criteria were 47 patients. Inclusion criteria were patients with type-2 DM who uses the same combination oral antidiabetic least three months. The effectiveness of the oral antidiabetic was assessed from the decline percentage fasting blood glucose (FBG) for three mounth. Cost and effectiveness analysis viewed based on the current average cost effectiveness ratio (ACER). The results showed metformin and glibenclamide are the most use of oral antidiabetic combination (68.08%). The medical direct cost of this oral antidiabetic combination is the cheapest (Rp.91.900). Metformin and glimepiride are the most effectiveness therapy for oral antidiabetic combination in type-2 diabetes out patient (7.47%). Based on the ACER value, the most cost-effective oral antidiabetic combination are metformin and glimepiride (Rp.15.000 per % effectiveness). Keywords: cost-effectiveness analysis, oral antidiabetic, type-2 DM
